California School Immunization Law

According to the California School Immunization Law, children must have their required immunizations (shots) before they can attend school or child care. Please use the following schedule to see if your child has all the required shots.
At this age: Child must have completed these shots:
Preschool 3 polio
4 DTaP (DTP, Td, or DT)
3 Hep—At least 1 Hib must be given on or after the first birthday
1 MMR—must be given on or after the first birthday
1 varicella
Kindergarten 4 polio
5 DTaP (DTP, Td, or DT)
3 Hep—At least 1 Hib must be given on or after the first birthday
3 Hep B
2 MMR—must be given on or after the first birthday
1 varicella
7th grade 4 polio
5 DTaP (DTP, Td, or DT)
3 Hep—At least 1 Hib must be given on or after the first birthday
3 Hep B
2 MMR—must be given on or after the first birthday
1 varicella
1 Td booster
