To participate in a team sport at CCA, a sports physical is required from a physician. The two main parts of a sports physical are the medical history of your student and the physical exam. Linked below is the Physical & Parent Consent Form that you will need your Doctor to fill out and sign. If you need a reference, Dr. Dennis O’Brien from the Gerken Family Chiropractic office is familiar with CCA and has done many physicals for our students over the years. The cost of his service is $25 per physical exam.
